Documentation to Support External Service Diagnosis

Request for school staff members to complete documentation/questionnaires to support external service diagnosis

Dear families if you are seeking input from your child’s classroom teacher in the form of observations, questionnaires or other forms we ask that you please provide the following information. The below information will support the classroom teacher in answering the questions in an informative manner.

• Type of form/questionnaire to be completed.
• Name of medical professional (please include the email address of the professional. Any form or document will be sent from the school directly to the medical professional)
• Challenges that you are experiencing/area of investigation.

Please allow a minimum of two weeks in order for any documentation to be returned.
